If you are a commercial truck operator with an International Registration Plan (IRP) or International Fuel Tax Agreement (IFTA) account, you cannot update your address on the standard MyDMV portal.
Commercial accounts are strictly regulated. To move your trucking business or home base, you must file Form HSMV 85041.
Important: If you are a standard driver looking to update your personal license, this is the wrong guide. Please use our Florida driver’s license address change guide instead.
Who Needs Form 85041? (Commercial Carriers Only)
This form is exclusively for:
- IRP Registrants: Carriers operating across state lines with apportioned plates.
- IFTA Licensees: Carriers filing quarterly fuel tax reports.
When to Use It:
- You moved your Established Place of Business.
- You moved your Residence (if you are an owner-operator based at home).
- You are changing your Mailing Address only.
The “3-Document Rule”: Proving Your New Business Location
If you are changing your Physical Address, you cannot just sign the form. You must prove you actually exist at that location to prevent fraud.
You must attach THREE documents from the lists below.
Option A: Established Place of Business
(For offices/warehouses)
- Mandatory: IRS form (W-2, W-3, or 941) proving you have paid employees.
- Plus Any Two (2) of the Following:
- Commercial Lease Agreement.
- Current Utility Bill (Electric/Water) in business name.
- Property Tax Assessment.
- Occupational/Business License from the city/county.
Option B: Residence
(For owner-operators working from home)
- Mandatory: Unexpired Florida Driver’s License.
- Plus Any Two (2) of the Following:
- Personal Federal Income Tax Receipt (IRS).
- Utility Bill or Bank Statement (Last 90 days).
- Property Tax Bill.
- Florida Vehicle Title or Registration.
How to Submit the Form (Email vs. Mail)
The submission method depends entirely on what you changed.
Scenario A: Mailing Address Change ONLY
- Method: Email.
- Process: Fill out the form, sign it, and email it to [email protected].
- Fee: None.
Scenario B: Physical Address Change
- Method: Mail Only. (You cannot email this because of the required proofs).
- Process: Mail the form + 3 Proof Documents to:Bureau of Commercial Vehicle and Driver Services 2900 Apalachee Parkway, MS 62 Tallahassee, FL 32399-0626
- Fee: You must pay a replacement fee (approx $4.00) for each vehicle in your fleet because every truck needs a new Cab Card with the new address.
